Introduction:

Hemolytic-uremic syndrome(HUS) is an important reason of acut kidney injury in children. Few data exists on the clinical features and long-term outcome of HUS in Turkey. The aim of this study was to describe the clinical, demographic, epidemiological and laboratory features and outcomes.

Material and methods:

In a retrospective multicenter study, 63 patients who have been diagnosed with HUS are evaluated in 26 centers of Pediatric Nephrology Departments of Turkey from January 2017 to February 2018. Data were analyzed by microsoft excell 2010.

Results:

63 patients, 35 female(%55.5) and 28 (%44.5) male  were enrolled the study. 61 (%96.8) were D(+)  and 2 (%3.1) were D(-) HUS. Age at disease onset  was  <5 years in 47 patients, 2 patients were younger than 6 month. There was enfection history in 14 patients (%22.2) and parents were consanguineus in 7 patients (%11.1). 37 patients(%58.7) were used antibiotics before the admission. 27 patients (%42.8) were anuric. 37 patients had nephrotic proteinuria and 13 had macroscopic hematuria. Neurological complications occured in 7 patients(%11.1). Cobalamin metabolism defect was detected in 1 patient. 32 (%50.8) patients needed ıntensıve care unit support. ADAM-TS 13 levels were normal in 51 patients. Plasma exchange was performed 20 (%31.7) patients, eculizumab therapy was given 18 (%28.6) patients. The mean duration of onset of eculizumab treatment was found to be 12,9 day. In total 44 patients (%69.8) needed dialysis, 12(%27.3) were periton dialysis and 32(%72.7) were hemodialysis. Renal biopsy was performed on 3 patients with TMA findings. 31 patients (%49.2) were seen to have applied after the religious holiday. 3 patients diagnosed with chronic renal disease and 2 patients (%4) died.

Conclusions:

HUS  patients had a high rate of complications and sequelae of kidney. Prospective trials are needed to prevent chronic kidney disease.
